- Current Pennsylvania Child Abuse History Certification

- Current Pennsylvania Criminal History Check
- Apply, check the status, or renew at https://epatch.pa.gov/home (No FEE FOR VOLUNTEERS)
- Current Federal Criminal History Record (Fingerprinting) or Volunteer Affidavit.
- Option A (PA residents for 10 or more years): Complete the Volunteer Affidavit from https://www.dhs.pa.gov (No FEE)

- Option A (PA residents for 10 or more years): Complete the Volunteer Affidavit from https://www.dhs.pa.gov (No FEE)
- PDE-6004 Arrest/Conviction Report and Certification Form (Acts 24 and 82)

- Verification of a Recent Tuberculosis Test less than three months old if you have never volunteered before.
- Ask your doctor or schedule a test at MedExpress ($47.00 from MedExpress or $35 test/ $39 follow-up visit from CVS without insurance coverage). After receiving results, scan/photograph the completed paperwork
Once you have submitted ALL of the above requirements you may begin to volunteer with the PTA. Our volunteers are room parents for classroom parties and help at various activities throughout the year during the school day and evening programs.
Remember to bring a state or federally issued Photo ID to enter the building. Volunteers without ID will not be allowed to enter beginning as of fall of 2023.
